Indiana Addiction Treatment Center Faces Scrutiny After Deaths and Calls for License Revocation

Following three recent deaths and numerous calls for its license revocation, an Indiana addiction treatment center has seen a shake-up in leadership. The executive director and director of nursing have departed from Praxis Landmark Recovery, a men's drug and alcohol rehabilitation facility located near South Bend, Indiana. The company confirmed the leadership changes to local news outlet WNDU-TV.

Landmark Recovery issued a statement on Friday assuring the public that other senior managers would provide continuous on-site supervision to ensure patient safety during this transition. The company also stated it would temporarily halt new patient admissions until a thorough staff review confirmed adherence to their standards.

Authorities report a series of troubling incidents at the facility since July 4th, including three patient deaths and another individual requiring multiple doses of Narcan for revival. This alarming pattern has prompted the St. Joseph County Police Department to formally request a state investigation and the revocation of the facility’s license.

According to Troy Warner, spokesperson for the county police, there have been over 200 calls to police and fire departments related to the 160-bed facility in 2023 alone. These incidents encompass a range of serious issues including nine overdoses, two rapes, a sexual assault, and a stabbing. In a letter to Jay Chaudhary, director of the Indiana Division of Mental Health and Addiction, Warner expressed deep concern, stating that the facility is straining county resources and endangering both patients and the wider community.

Landmark Recovery, which operates facilities in ten states and is headquartered in Tennessee, revealed that the Joint Commission, a national accrediting agency, recently conducted a site visit at the Indiana center, which opened in 2022. While the complete report is pending, Landmark Recovery stated that preliminary findings validate their care model and expressed anticipation for sharing the full report in the future.
